Course Description:

'' Software has become pervasive in today’s complex systems, and safety-critical systems are no exception. Recent losses such as the Ariane 5 rocket, which result from unexpected software behavior or system interactions, highlight the need for a more rigorous approach to software engineering that focuses on the safety-critical nature of these systems. This course explores the challenges facing engineers in the development and operation of software for safety-critical systems. It is geared toward software and systems engineers who would like a more in-depth understanding of system safety engineering and training in a more rigorous approach to safety-critical software engineering. Participants are first exposed to the theory underlying design-for-safety approaches, such as accident modeling and hazard analysis techniques. A survey of accidents and their causation from a variety of domains, including aerospace, nuclear, and medicine, is presented and discussed. An accident analysis activity is completed by the class to better understand accident modeling, why accidents happen, and what can be done to prevent them. Next, the process of designing software with respect to safety is presented, following the typical software engineering lifecycle. Several strategies that can be employed to aid design-for-safety efforts, such as formal modeling and analysis, human factors engineering, and creating a safety culture, also are discussed. Participants apply the design-for-safety approaches they learn in class to an example safety-critical software control system. Several industry standards for safety-critical software also are introduced. Finally, state-of-the-art accident modeling and hazard analysis techniques that utilize systems theory are presented and discussed. By the end of this course, participants should be able to understand accident modeling and analysis; understand the differences and relationships between safety, reliability, and risk; perform several hazard analysis techniques; use design-for-safety strategies in their software development efforts; and recognize the current standards and research in safety-critical software systems. ...''
